What is a MILA tummy tuck?

MILA — Minimally Invasive Lipo-Abdominoplasty — is Dr. Paulo Michels’ signature technique for the patient who needs the muscle and fat corrected but whose skin will retract on its own. The crucial point most people miss is that MILA does the same muscle repair as a tummy tuck. The separated “six-pack” muscles (diastasis recti, often stretched apart by pregnancy) are stitched back together in a complete plication, rebuilding a firm internal corset.

What makes it minimally invasive is the access. That repair is delivered through the same millimetre-sized incisions used in liposuction, not through a long open cut. VASER ultrasound emulsifies and sculpts the fat for a flat base, and plasma energy helps the overlying skin retract and firm. What is a full tummy tuck?

MILA vs full tummy tuck: how do they actually differ?

Both operations repair the muscle in exactly the same way. The two real differences are what happens to the skin, and therefore the scar.

  • Skin removal. MILA removes none and relies on the skin retracting; a full tummy tuck physically excises the loose skin.
  • Scar. MILA scars like liposuction — a few discreet millimetre ports. A full tummy tuck leaves a low, hip-to-hip scar.
  • Who it suits. MILA is for muscle separation and fat with mild skin laxity. A full tummy tuck is for muscle separation and fat with moderate-to-severe skin laxity.
  • Recovery. Because no skin is excised, MILA recovery is closer to advanced liposuction, though the internal muscle repair still has to be respected exactly as it would after a tummy tuck.

MILA is powerful, but it has one honest limit: it does not remove skin. When there is a moderate-to-severe amount of loose skin, that skin has to be excised, and a full tummy tuck is the truthful answer. The whole plan turns on how much your skin will retract — which is why the technique is never chosen from a photo.

Which one is right for you?

It comes down to your skin, not your preference. If you have muscle separation and fat to correct but your skin quality is good and the laxity is mild, MILA can give you the muscle repair of a tummy tuck without its scar.
