Fleur-de-Lis Tummy Tuck
Medically reviewed by Dr. Paulo Michels, Board-Certified Plastic Surgeon· Updated 22 October 2026

A fleur-de-lis tummy tuck is an abdominoplasty that adds a vertical midline incision to the usual low horizontal one, so loose skin can be removed both from top to bottom and from side to side. It suits patients with wide, folded abdominal skin after massive weight loss, bariatric surgery or GLP-1 medicines, or with an existing midline scar. Dr. Paulo Michels performs it at Elyzee Hospital in Abu Dhabi for patients from Dubai and across the UAE.
A standard tummy tuck pulls loose skin downwards and removes it through a low scar. That works well when the excess runs from top to bottom. After massive weight loss, though, the skin is often loose in both directions: it hangs down, and it is also too wide, with vertical folds and a waist that stays broad however firmly the skin is pulled down. For this pattern, the fleur-de-lis tummy tuck adds a vertical excision down the middle of the abdomen.
Dr. Paulo Michels performs the fleur-de-lis abdominoplasty at Elyzee Hospital in Abu Dhabi, mainly for patients after bariatric surgery, GLP-1 weight loss or large weight loss through diet, and for patients who already have a vertical midline scar.
What is a fleur-de-lis tummy tuck?
The operation removes two wedges of skin at once: a horizontal wedge low on the abdomen, as in a standard tummy tuck, and a vertical wedge down the midline. Seen flat, the skin removed resembles the fleur-de-lis, the French lily — hence the name. The closed scars form an inverted T.
- The horizontal part removes excess from top to bottom
- The vertical part removes excess from side to side, narrowing the waist and flattening vertical folds
- The midline incision also gives direct access for muscle repair along the full length of the abdomen
Patients also search for it as a vertical tummy tuck, a T tummy tuck or an inverted-T abdominoplasty.
Who is it for?
- After massive weight loss — skin that is loose across the whole front, with vertical creases and a wide waist
- After bariatric surgery — once weight and nutrition are stable
- After GLP-1 weight loss — semaglutide (Ozempic, Wegovy) or tirzepatide (Mounjaro); usually paused about two weeks before surgery as directed by the anaesthesia team, never on your own
- An existing vertical midline scar — from earlier open abdominal surgery; the old scar can be included in the skin removed
- A wide upper abdomen — looseness above the belly button that a low incision cannot tighten sideways
Weight should be stable for several months before surgery. If the skin is loose mainly from top to bottom, a standard tummy tuck gives a good result without the vertical scar, and that is what Dr. Michels recommends. The broader picture is described in loose skin after weight loss.
The trade-off: the vertical scar
The vertical scar is the price of the operation. It runs down the middle of the abdomen and is visible in a bikini or low-rise clothing, whereas the horizontal scar sits within underwear. The two scars meet in a T-junction, which is the area most prone to delayed healing because its blood supply is the most limited.
For a patient with marked side-to-side excess, the narrower waist and smoother front usually outweigh the scar. For a patient with modest width, they do not. The decision is made together at the consultation, with you standing in front of a mirror, so you can see exactly what the vertical excision would remove.
Fleur-de-lis compared with other tummy tucks
| Procedure | Scar | Treats looseness | Best for |
|---|---|---|---|
| Standard (full) tummy tuck | Low horizontal, hip to hip | Top to bottom, below and around the belly button | Post-pregnancy laxity and muscle separation |
| Extended tummy tuck | Low horizontal, continued onto the flanks | Top to bottom, front and flanks | Loose skin reaching the hips |
| 360 tummy tuck | Belt all the way around the waist | Top to bottom, front, flanks and back | Excess that wraps round to the back |
| Reverse tummy tuck | In the breast fold | Upper abdomen, lifted upwards | Loose skin above the navel with a firm lower abdomen |
| Fleur-de-lis tummy tuck | Low horizontal plus vertical midline (inverted T) | Top to bottom and side to side | Wide, folded skin after massive weight loss, or an existing midline scar |
These operations can be combined. A patient with excess in every direction may need a circumferential operation with a vertical component; another may need a fleur-de-lis now and the arms or thighs later. The examination decides.
How is a fleur-de-lis tummy tuck performed?
The operation is performed under general anaesthesia at Elyzee Hospital.
- Marking — with you standing and lying, the horizontal and vertical excisions are marked, testing how much skin can be removed in each direction without excessive tension.
- Horizontal excision — the lower abdominal skin is lifted and the excess removed, as in a full tummy tuck.
- Muscle repair — through the midline, separated rectus muscles (diastasis recti) are brought together along their length; small midline hernias are repaired.
- Vertical excision — the midline wedge is removed, narrowing the abdomen and flattening vertical folds.
- Belly button — the belly button is brought out along the vertical line and shaped to look natural; when the original is stretched or damaged, a new one can be created.
- Closure — the scars are closed in layers, with particular care at the T-junction.
Liposuction of the flanks or waist can refine the contour, but it is limited close to the incisions, because the skin flaps depend on a good blood supply.
Pain and recovery
Every abdominal procedure in this practice includes ultrasound-guided nerve blocks, placed by the anaesthesiologist before you wake, which reduce early pain and the need for strong painkillers.
- Days 1–3 — short hospital stay; walking from the first day
- Weeks 1–2 — rest at home; Manual Lymphatic Drainage (MLD) begins
- Weeks 2–3 — most patients return to desk work
- Around week 6 — compression garment off
- Weeks 6–8 — gym and heavier lifting once the muscle repair has healed
- Months 12–18 — scars mature and fade
Progressive tension sutures can close the space where fluid collects and, in limited cases, allow a drainless recovery; after more extensive surgery a drain may be used for a few days. Walking early and other measures chosen after your individual risk assessment reduce the risk of blood clots. Not smoking and good nutrition matter especially for healing at the T-junction. In the UAE climate, stay in air-conditioned spaces while wearing the garment and keep the scars out of direct sun for the first year.

Questions, answered
Fleur-de-Lis Tummy Tuck in Abu Dhabi — your questions
What is a fleur-de-lis tummy tuck?+
A fleur-de-lis tummy tuck, or fleur-de-lis abdominoplasty, combines the standard low horizontal tummy tuck incision with a vertical incision down the middle of the abdomen. The horizontal part removes excess skin from top to bottom; the vertical part removes excess from side to side, narrowing the waist. The combined scar resembles an inverted T, and the name comes from the shape of the skin removed, which looks like the French lily.
Who needs a fleur-de-lis tummy tuck instead of a standard tummy tuck?+
Patients whose loose abdominal skin is excessive in width as well as in height. After massive weight loss the skin is often slack across the whole front, with vertical folds and a waist that stays wide even after a standard tummy tuck pulls it down. The vertical excision removes that horizontal excess, which a low scar alone cannot reach. When the skin is loose mainly from top to bottom, a standard tummy tuck is enough.
What is a vertical tummy tuck?+
Vertical tummy tuck is another name patients use for a fleur-de-lis abdominoplasty, because of the vertical midline scar. It is sometimes also called a T tummy tuck or an inverted-T abdominoplasty. The vertical incision is almost always combined with a horizontal one; a purely vertical excision is used only in particular situations, such as when a long vertical scar already exists.
Is a fleur-de-lis tummy tuck suitable after bariatric surgery or GLP-1 weight loss?+
Often, yes — these are the patients for whom it was designed. After bariatric surgery or weight loss with semaglutide (Ozempic, Wegovy) or tirzepatide (Mounjaro), the skin has been stretched in every direction and does not retract. Surgery is planned once weight has been stable for several months; GLP-1 medicines are usually paused about two weeks before surgery as directed by the anaesthesia team, never on your own.
I already have a vertical scar from previous surgery. Is fleur-de-lis a good option?+
Frequently it is the logical option. If an open operation — for example an earlier abdominal or bariatric procedure — has left a vertical midline scar, the fleur-de-lis design can include that scar in the skin that is removed. You end up with a new, neater vertical scar in place of the old one, and the extra width of skin is treated at the same time.
What is the main disadvantage of a fleur-de-lis tummy tuck?+
The vertical scar. It runs down the middle of the abdomen and is visible in a bikini or low-rise clothing, unlike the horizontal scar, which is hidden in underwear. For patients with marked side-to-side excess, the improvement in waist and contour usually outweighs the scar, but it is a personal decision made with clear information at the consultation.
Where are the scars after a fleur-de-lis tummy tuck?+
There is a low horizontal scar across the front, as in a standard tummy tuck, and a vertical scar in the midline from the upper abdomen down to meet it, forming an inverted T. The belly button is brought out along the vertical line. Both scars are closed in layers and usually fade over 12 to 18 months, but the vertical scar remains visible.
What happens to the belly button in a fleur-de-lis tummy tuck?+
The belly button stays attached to the abdominal wall and is brought out through the skin at its new position along the vertical closure, then shaped so it looks natural. When the original navel is stretched or damaged, a new one can be created. Its final look is discussed at the consultation.
Are the abdominal muscles repaired in a fleur-de-lis tummy tuck?+
Yes, when they are separated. The vertical incision gives direct access to the whole midline, from the breastbone to the pubic area, so separated rectus muscles (diastasis recti) and small midline hernias can be repaired in the same operation. If a larger hernia is present, the plan is coordinated with a general surgeon.
How is a fleur-de-lis different from an extended or a 360 tummy tuck?+
An extended tummy tuck lengthens the horizontal incision onto the flanks; a 360 tummy tuck continues it all the way around the back. Both treat looseness from top to bottom and around the body. A fleur-de-lis adds a vertical excision to treat looseness from side to side across the front. The approaches can be combined — a circumferential operation with a vertical component — when the excess is in every direction.
Can a fleur-de-lis tummy tuck be combined with liposuction?+
Yes, with care. Liposuction of the flanks or waist can refine the contour, but the skin flaps in a fleur-de-lis rely on a good blood supply, so liposuction close to the incisions is limited. The amount is decided case by case.
Can it be combined with other surgery?+
Sometimes with a breast lift, but after massive weight loss the larger plan is usually staged: the abdomen first, then the breasts, arms or thighs in a later operation. Staging keeps each anaesthetic shorter and each recovery more manageable.
What are the risks of a fleur-de-lis tummy tuck?+
Besides the risks of any abdominal operation — bleeding, infection, fluid collection (seroma), blood clots and anaesthetic risk — the specific concern is delayed healing where the vertical and horizontal scars meet, because the skin at that junction has the most limited blood supply. A visible or widened vertical scar is the other trade-off. Not smoking, stable nutrition and careful design reduce these risks.
How painful is a fleur-de-lis tummy tuck?+
Comparable to a full tummy tuck with muscle repair, with tightness along both scars. Ultrasound-guided nerve blocks are placed by the anaesthesiologist before you wake, which markedly reduces the early pain and the need for strong painkillers, and helps you walk sooner.
How long is recovery after a fleur-de-lis tummy tuck?+
Expect a short hospital stay, walking from the first day and one to two weeks of rest at home. Most patients return to desk work at around two to three weeks, wear the compression garment for about six weeks and return to the gym at six to eight weeks once the muscle repair has healed. Manual Lymphatic Drainage starts in the first week.
Will I need drains?+
It depends on the extent of dissection and liposuction. Progressive tension sutures can close the space where fluid collects and, in limited cases, allow a drainless recovery; after more extensive operations a drain may be used for a few days.
How long do the results last?+
The removed skin does not return, so the result is long-lasting as long as your weight stays stable. Ageing continues, and a significant weight regain or a pregnancy can stretch the abdomen again.
How much does a fleur-de-lis tummy tuck cost in Abu Dhabi or Dubai?+
The cost depends on the extent of the excision, any hernia repair, liposuction or combined procedures, and the hospital stay. Fees are quoted individually after the consultation and include the surgeon, the anaesthesiologist with nerve blocks, the hospital and theatre, compression garments, discharge medication and follow-up. No fixed price list is published.
Do you perform fleur-de-lis tummy tucks for patients from Dubai?+
Yes. Many of Dr. Michels' body-contouring patients live in Dubai. All consultations and surgery take place at Elyzee Hospital in Abu Dhabi, and pre-operative tests, surgery and follow-up visits are grouped where possible to reduce the number of journeys.
