Minimally Invasive vs Classic Breast Augmentation: Which One Suits You?

Minimally invasive breast augmentation has attracted a great deal of attention in Dubai and Abu Dhabi — a short procedure, a small scar in the armpit and a promise of returning to normal life within a day or two. Many women now ask whether it is the “new standard” and whether classic augmentation is outdated.

The honest answer is that the two approaches are designed for different women. This guide explains what each one is, who each suits, and the questions worth asking before you choose. It is written to inform, not to criticise any technique or any colleague.

What is minimally invasive breast augmentation?

The best-known system is Mia Femtech, developed by Establishment Labs, the company behind Motiva breast implants. In the published technique, a small, lightweight implant is placed through a short incision in the armpit, in front of the chest muscle, using a dedicated insertion system. It is usually performed under sedation with local anaesthesia and takes well under an hour.

Its aim is a subtle, natural increase — often described as one to two cup sizes — rather than a change of shape. Patients typically go home the same day, and recovery is lighter than after a classic operation.

What is classic breast augmentation?

Classic augmentation is the operation that has been refined over decades and adapted to almost every anatomy. At Elyzee Hospital in Abu Dhabi, Dr. Paulo Michels performs breast augmentation with:

  • an incision hidden in the breast fold, giving direct control of the fold and lower pocket;
  • Dual Plane placement as standard, so the upper part of the implant is covered by muscle;
  • cohesive silicone implants from Motiva, Mentor or Polytech, in any size and profile your chest can safely carry;
  • no-touch insertion with the Keller Funnel in every implant operation;
  • the Internal Bra technique — permanent sutures reinforcing the fold so the implant does not drop over the years;
  • Crisalix 3D simulation at the consultation to preview size and shape.

It is performed under general anaesthesia with ultrasound-guided nerve blocks for comfort.

Side by side

Minimally invasive (e.g. Mia) Classic augmentation
Typical increase Modest, about 1–2 cup sizes Any size your chest can safely carry
Incision Short incision in the armpit Breast fold (also areola or armpit)
Implant position In front of the muscle Dual Plane, or other planes as needed
Implant choice One implant family, small volumes Full range of brands, shapes and profiles
Sagging breasts Not suitable Can be combined with a lift
Asymmetry, tuberous breasts Excluded in the published study Can be corrected
Previous implants / revision Not suitable Routine
Anaesthesia Sedation and local General, with nerve blocks
Evidence One three-year study, manufacturer-funded Decades of multicentre data

Who suits the minimally invasive approach?

The main published study of the Mia technique (Aesthetic Surgery Journal) enrolled women with a normal body-mass index, a breast width under 14 cm and no sagging, and excluded tuberous breasts, marked asymmetry and previous breast surgery. That is also the profile surgeons offering it describe: a young woman with well-shaped breasts who wants a little more volume and a very short recovery.

If that is you, it is a reasonable option to discuss — and a good surgeon will also tell you if your goal is small enough to leave things as they are.

When is classic augmentation the better fit?

Most women who come for breast surgery do not fit the narrow profile above. Classic augmentation is the more suitable choice when:

What does the evidence say?

The three-year results of the main Mia study are encouraging: low complication and reoperation rates, no capsular contracture or rupture reported, and high satisfaction. Its authors also note its limits — 100 patients, one centre, one surgeon — and the study was funded by the manufacturer, which the authors disclose.

According to the manufacturer, the Mia system and its Ergonomix2 Diamond implant are not cleared for sale in the United States; they carry a European CE mark. Other Motiva implants received FDA approval in September 2024.

In short: early results are promising, and longer, independent follow-up will show how the technique performs over the life of an implant. Classic augmentation rests on decades of data across many centres.

Does recovery differ?

Yes. A minimally invasive procedure under sedation, with a small implant in front of the muscle, is usually lighter to recover from. After classic augmentation you can expect tightness for the first days, light walking from day one, back-sleeping for 4 to 6 weeks and no high-impact exercise or upper-body weights for 4 to 6 weeks — see breast augmentation recovery week by week. Faster recovery is a real advantage, but it should be weighed against what the operation can achieve for your anatomy.

Questions to ask any surgeon

  • Am I a candidate for this technique, or is my breast shape outside its indications?
  • How much larger can I realistically go, and will it look proportionate on me?
  • Where will the implant sit, and how much tissue will cover it?
  • If I ever need revision, how will it be done — and through which incision?
  • Which implant will you use, and what is its regulatory status?
  • Can I see a 3D simulation and results in patients with a similar build?
