Auto-Augmentation Breast Lift (Breast Lift Without Implants)
Medically reviewed by Dr. Paulo Michels, Board-Certified Plastic Surgeon· Updated 22 October 2026

An auto-augmentation breast lift is a breast lift without implants: instead of discarding the lower breast tissue, the surgeon moves it upwards as a flap beneath the breast to restore upper fullness, then supports it with an Internal Bra built from the patient's own tissue. It suits women with enough breast tissue who do not want an implant. Dr. Paulo Michels performs it at Elyzee Hospital in Abu Dhabi for patients from Dubai and across the UAE.
A breast lift raises the nipple, removes stretched skin and reshapes the breast — but on its own it does not create upper fullness, and many women assume the only way to get that is an implant. It is not. In an auto-augmentation breast lift, the lower breast tissue that a lift would often remove is kept, moved upwards and used to fill the upper breast: a fuller, lifted breast made entirely of your own tissue.
Dr. Paulo Michels performs the auto-augmentation mastopexy at Elyzee Hospital in Abu Dhabi and secures it with his autologous Internal Bra, so the result is supported from within rather than by skin alone.
Who is a breast lift without implants for?
The operation suits women with sagging breasts who still have enough breast tissue and do not want an implant:
- After pregnancy and breastfeeding — the breast has dropped and emptied at the top, but tissue remains in the lower breast
- After weight loss — when enough breast tissue is left to reshape
- After breast implant removal — to reshape a breast stretched by an implant and restore upper fullness without a new one
- Preference for no foreign body — women who want a natural result and no future implant revisions
It is less suitable when there is very little breast tissue or when a clear increase in size is wanted — in that case a breast lift with implants or added fat transfer is discussed. The degree of sagging is graded first; see sagging breasts (ptosis).
How does an auto-augmentation breast lift work?
The operation is performed under general anaesthesia at Elyzee Hospital, with ultrasound-guided nerve blocks.
- Marking — with you standing, the new nipple position, the skin to be removed and the flap of lower breast tissue are marked.
- Creating the flap — a section of lower breast tissue is kept attached to the chest wall so it keeps its blood supply.
- Repositioning — the flap is moved upwards beneath the breast and fixed to the chest wall, where it fills the upper breast.
- The Internal Bra — a sling of your own fascia and muscle is secured beneath the reshaped breast, and permanent sutures reinforce the breast fold, so the lift is carried by deep structures rather than skin.
- Nipple, areola and skin — the nipple and areola are moved higher, stretched areolas are reduced, and the skin is tightened and closed with surgical glue.
Volume is redistributed, not added: the breast is not larger overall, but the fullness sits higher, so it looks rounder and more projected in clothing.
Auto-augmentation, lift with implants or fat transfer?
| Auto-augmentation breast lift | Breast lift with implants | Fat transfer | |
|---|---|---|---|
| Lifts the nipple and tightens skin | Yes | Yes | No |
| Upper fullness from | Your own repositioned breast tissue | Silicone implant | Your own fat |
| Overall volume | Redistributed, not increased | Increased | Modestly increased |
| Foreign body | None | Implant | None |
| Needs | Enough breast tissue | Wish for more volume | Enough donor fat, little sagging |
| Scars | Vertical or anchor | Vertical or anchor | Tiny entry points |
| Best for | Sagging with enough tissue, no implant wanted | Sagging with marked volume loss | Subtle increase without sagging |
The three can also be combined: fat transfer is often added to an auto-augmentation to soften the upper contour. The broader decision is explained in breast lift or implants?.
What scars will I have?
The flap is built from the lower breast, so the incision must give access to it. The pattern follows the degree of sagging:
| Sagging | Usual pattern | Scar |
|---|---|---|
| Moderate (grade 2) | Vertical (lollipop) | Around the areola and down to the breast fold |
| Severe (grade 3) | Anchor (inverted T) | Around the areola, vertical, and along the breast fold |
A periareolar (around the areola only) lift does not give enough access for an auto-augmentation. Scars are permanent but usually fade and flatten over 12 to 18 months; the patterns are compared in breast lift incision patterns explained.
Nipple and areola corrections in the same operation
A lift always repositions the nipple and areola, and other concerns can be treated at the same time:
- Stretched areolas are reduced to suit the new breast — see large or stretched areolas
- Large or long nipples can be reduced in height or width — see large nipples
- Inverted nipples can be released — see inverted nipples
Dr. Michels performs these nipple and areola corrections only as part of a breast operation such as a lift, never as isolated procedures.
Breast lift without implants after explant
After breast implant removal, the skin has often been stretched by the implant and the upper breast looks empty. If enough breast tissue remains, an explant-lift with auto-augmentation and the Internal Bra reshapes the breast and restores upper fullness without a new implant. Where the tissue is thin, fat transfer can be added in the same operation.
Pain and recovery
Ultrasound-guided nerve blocks keep the first days comfortable, and because there is no implant under the muscle, the deep chest soreness of an augmentation is usually absent. Dr. Michels’ breast lifts are usually performed without drains.
- Day of surgery — walking the same day; support bra on
- Days 5–7 — most patients return to desk work and driving once off prescription painkillers
- Weeks 3–4 — light cardio
- Weeks 4–6 — support bra day and night until cleared; then full exercise
- Months 3–6 — the breast settles into its final shape
- Months 12–18 — scars mature and fade
Sleep on your back for the first weeks, keep the scars out of direct sun for the first year, and stay in air-conditioned spaces in the UAE heat.
What are the risks?
The risks are those of any breast lift — bleeding, infection, delayed wound healing, changes in nipple sensation, asymmetry and visible scars. Specific to the flap are areas of firmness if part of the repositioned tissue does not heal well, and some loss of upper fullness over time. Any breast surgery can affect the ability to breastfeed. Careful flap design, secure fixation and the Internal Bra are what keep these risks low.
Patients from Dubai
Many of Dr. Michels’ breast-lift patients live in Dubai. All consultations and surgery take place at Elyzee Hospital in Abu Dhabi — you can book by WhatsApp or phone on +971 50 106 7981, or through the contact form. The pre-operative tests, surgery and follow-up visits are grouped where possible to keep the number of journeys down. See also breast lift for patients in Dubai.
What does your investment include?
Auto-augmentation breast lift fees are quoted individually after consultation, because the degree of sagging and any combined procedure — fat transfer, nipple or areola correction, a reverse tummy tuck — determine the operating time and hospital stay. The quote is all-inclusive:
- Surgeon’s fee and pre-operative planning
- Specialist anaesthesiologist, including ultrasound-guided nerve blocks
- Hospital stay and operating theatre at Elyzee Hospital
- Post-operative support garments
- Discharge medication
- Follow-up appointments and ongoing clinical support
Questions, answered
Auto-Augmentation Breast Lift (Breast Lift Without Implants) in Abu Dhabi — your questions
What is an auto-augmentation breast lift?+
An auto-augmentation breast lift, or auto-augmentation mastopexy, is a breast lift that uses your own breast tissue to create upper-breast fullness. In a standard lift, some tissue from the lower breast is often removed. In an auto-augmentation, that lower tissue is kept as a flap with its own blood supply, moved upwards beneath the breast and secured to the chest wall, so it fills the upper breast like a small implant made of your own tissue.
Can a breast lift be done without implants?+
Yes. Many breast lifts are performed without implants: the skin is tightened, the breast tissue reshaped and the nipple raised. An auto-augmentation goes one step further by repositioning your own lower breast tissue to the upper breast, so the lifted breast has more upper fullness than a lift alone would give. Whether you need an implant depends on how much breast tissue you have and how full you want the result to be.
Who is a good candidate for a breast lift with own tissue?+
The ideal candidate has sagging breasts with enough remaining breast tissue, especially in the lower breast, wants a lifted and fuller upper breast, and prefers not to have an implant. Typical situations are sagging after pregnancy and breastfeeding, after weight loss, and after breast implant removal. A stable weight and having finished having children make the result last longer.
Who is not a good candidate for auto-augmentation?+
Women with very little breast tissue, or who want a clear increase in size, usually need an implant or a hybrid approach, because the operation only reshapes the tissue you already have; it does not add new volume. Active smokers, patients still losing weight and women planning a pregnancy soon are also advised to wait or choose a different plan.
How is the auto-augmentation flap created?+
During the lift, a section of the lower breast tissue that would otherwise be removed is kept attached to the chest wall so it keeps its blood supply. It is then moved upwards beneath the reshaped breast and fixed in position. The breast skin and remaining tissue are tightened over it, and the nipple and areola are moved to their new, higher position.
How does the Internal Bra support an auto-augmentation?+
Dr. Michels secures the repositioned tissue with his Internal Bra technique: a sling of the patient's own fascia and muscle from the chest wall, plus permanent internal sutures that reinforce the breast fold. The lifted breast and the flap rest on living tissue rather than on skin alone, which helps the upper fullness and the lift to hold over time. No mesh or scaffold material is needed.
Will an auto-augmentation make my breasts bigger?+
Not in overall volume — the tissue is your own and it is redistributed, not added. What changes is where the volume sits: it moves from the lower breast to the upper breast, so the breast looks fuller, rounder and more projected in clothing. If you want a larger cup size, an implant or fat transfer would be needed in addition.
What is the difference between a breast lift with implants and an auto-augmentation?+
A breast lift with implants adds volume with a silicone implant and lifts the breast in the same operation; it suits women who have lost a lot of volume or want to be larger. An auto-augmentation adds no implant and uses your own tissue for upper fullness; it suits women who have enough tissue and want to avoid a foreign body. Both lift the nipple and remove stretched skin.
Can fat transfer be combined with an auto-augmentation breast lift?+
Yes. When a little extra volume or a softer upper contour is wanted, fat harvested by liposuction can be grafted at the same operation. This keeps the result entirely natural, with no implant, although part of the transferred fat is reabsorbed and the gain from fat is modest.
What scars does a breast lift without implants leave?+
The scars depend on how much the breast has dropped. Because the flap is built from the lower breast, an auto-augmentation needs a vertical (lollipop) pattern — around the areola and down to the fold — or an anchor pattern, which adds a scar hidden in the breast fold. Moderate sagging is usually treated with a vertical scar; severe (grade 3) sagging needs the anchor pattern. Scars fade and flatten over 12 to 18 months.
Can nipple and areola concerns be corrected in the same operation?+
Yes. The lift moves the nipple and areola higher, and stretched areolas are reduced to suit the new breast. Large or long nipples and inverted nipples can also be corrected during the same operation. Dr. Michels performs these nipple and areola corrections only together with a breast operation such as a lift, never on their own.
Is an auto-augmentation a good option after breast implant removal?+
Often, yes. After an explant, many women have sagging and an empty upper breast. If enough breast tissue remains, an explant-lift with auto-augmentation and the Internal Bra can reshape the breast and restore upper fullness without placing a new implant. When tissue is thin, fat transfer may be added, and the options are compared at the consultation.
Can I have an auto-augmentation after weight loss or Ozempic?+
It depends on how much breast tissue remains. After major weight loss, including weight loss with semaglutide or tirzepatide, some women still have enough tissue for an auto-augmentation; others have too little and need an implant for the fullness they want. Surgery is planned once weight has been stable for several months, and GLP-1 medicines are usually paused about two weeks before surgery as directed by the anaesthesia team — never on your own.
How long do the results of a breast lift without implants last?+
The lift is long-lasting, and supporting it with the Internal Bra helps the shape hold. The breast will still age, and pregnancy, breastfeeding and significant weight change can stretch it again. With no implant, there is no implant-related revision to plan for in the future.
Can I breastfeed after an auto-augmentation breast lift?+
Many women can, because the nipple usually remains attached to the underlying breast tissue in a lift. However, any breast surgery can reduce the ability to breastfeed, and this cannot be guaranteed. If future breastfeeding matters to you, say so at the consultation, and ideally plan the lift after you have finished having children.
How painful is the recovery?+
Most patients describe tightness and soreness rather than sharp pain. Ultrasound-guided nerve blocks placed by the anaesthesiologist keep the first days comfortable. Because there is no implant under the muscle, the deep chest soreness typical of an augmentation is usually absent.
How long is the recovery after a breast lift without implants?+
Most patients return to desk work in about 5 to 7 days, wear a support bra day and night for around 4 to 6 weeks, start light cardio after 3 to 4 weeks and return to full exercise at around 6 weeks. The breast settles into its final shape over 3 to 6 months.
What are the risks of an auto-augmentation mastopexy?+
The risks are those of any breast lift — bleeding, infection, delayed wound healing, changes in nipple sensation, asymmetry and visible scars — plus, specific to the flap, firmness if part of the repositioned tissue does not heal well, and some loss of upper fullness over time. Careful flap design, secure fixation and the Internal Bra are what reduce these risks.
How much does an auto-augmentation breast lift cost in Abu Dhabi or Dubai?+
The cost depends on the degree of sagging, whether fat transfer, a reverse tummy tuck or another procedure is combined, and the hospital stay. Fees are quoted individually after the consultation, once the plan is defined, and the quote is all-inclusive. No fixed price list is published.
Do you perform breast lifts without implants for patients from Dubai?+
Yes. Many of Dr. Michels' breast-lift patients live in Dubai. All consultations and surgery take place at Elyzee Hospital in Abu Dhabi, and the pre-operative tests, surgery and follow-up visits are grouped to keep the number of journeys to a minimum.
