Fat Transfer Breast Augmentation
Medically reviewed by Dr. Paulo Michels, Board-Certified Plastic Surgeon· Updated 22 October 2026

Fat transfer breast augmentation enlarges the breasts with the patient's own fat instead of an implant: fat is harvested by gentle liposuction, purified and injected in many fine layers into the breast. It suits women who want a modest, natural gain, have enough donor fat and reasonably firm skin. Dr. Paulo Michels performs it at Elyzee Hospital in Abu Dhabi for patients from Dubai and across the UAE.
Many women want slightly fuller breasts but do not want an implant — because they prefer to avoid a foreign body, because they have had implants removed, or simply because they want the change to be subtle. For them, fat transfer breast augmentation offers a natural alternative: your own fat, taken from an area where you have too much, is grafted into the breast.
Dr. Paulo Michels performs fat transfer breast augmentation at Elyzee Hospital in Abu Dhabi, alone or combined with a breast lift, implant removal or other body contouring. The most important part of the consultation is honest expectation-setting: fat gives a modest, natural gain, not a dramatic one.
Who is fat transfer breast augmentation for?
Natural breast augmentation with fat works best when the goal and the anatomy match its strengths:
- A modest increase — you want a fuller, softer breast or upper breast, not a large change in cup size
- Enough donor fat — the abdomen, flanks, back or thighs have fat that can be harvested, and you would welcome contouring there
- Good skin and position — the skin is reasonably firm and the nipple sits at or above the breast fold
- After implant removal — you want to soften the upper breast and recover some volume without a new implant
- Fine-tuning — mild asymmetry or a localised hollow that needs a little volume in a precise place
- Stable weight — your weight has been stable for several months and is expected to stay so
It is less suitable for women who want a substantial increase, who are very slim with little donor fat, or whose breasts have dropped significantly — in that case a lift is needed (see sagging breasts).
How is fat transfer to the breast performed?
The operation is performed under general anaesthesia at Elyzee Hospital, with ultrasound-guided nerve blocks placed by the anaesthesiologist.
- Planning and marking — with you standing, the donor areas and the zones of the breast to be grafted are marked; the two breasts are assessed separately.
- Harvesting — fat is collected by gentle liposuction, using VASER ultrasound-assisted or conventional suction depending on the area, with techniques that protect the fat cells. The donor area is contoured at the same time.
- Processing — the fat is purified to separate viable fat cells from fluid and oil.
- Grafting — through small entry points, the fat is injected with fine cannulas in many small threads, at different depths and in different directions, so that every portion lies close to a blood supply.
- Closing — the tiny entry points are closed and a compression garment is placed on the donor areas.
The breast is never over-filled. Packing too much fat into one place is the main cause of fat that does not survive, firm lumps and oil cysts — a conservative graft gives a better and safer result.
How much volume can I expect?
This is the question that matters most, and the answer is honest: a modest, natural increase per session. Two factors limit the result:
- What the breast can receive. The skin and tissue can only accommodate a certain amount of fat safely in one session; forcing more in reduces survival.
- Reabsorption. A portion of the transferred fat does not establish a new blood supply and is reabsorbed by the body over the first months. This is expected and part of the planning.
The breasts look larger in the first weeks because of swelling and fat that will not survive; the result is judged after several months. Women who want more than one session can give may choose a second session once the first has settled. Fat that survives behaves like the rest of your body fat — it changes if your weight changes.
Fat transfer, implants or hybrid augmentation?
| Fat transfer | Implant augmentation | Hybrid (implant + fat) | |
|---|---|---|---|
| Volume gain | Modest | Moderate to large, chosen in advance | Moderate to large, with a softer finish |
| Predictability | Less predictable; part of the fat is reabsorbed | High | High for the implant, fine-tuning with fat |
| Look and feel | Entirely natural tissue | Natural with good coverage | Natural; fat camouflages implant edges |
| Foreign body | None | Implant | Implant |
| Scars | Tiny entry points | Short scar in the breast fold | Short fold scar plus entry points |
| Donor fat needed | Yes | No | Yes, a smaller amount |
| Possible extra session | Sometimes | Rarely, for revision | Rarely |
| Best for | Subtle increase, no implant, after explant | Clear increase in size | Slim patients with thin coverage |
For a clear increase in size, see breast augmentation. For the combination of a smaller implant with fat, see what is hybrid breast augmentation?. None of these options lifts a breast that has dropped — that requires a lift, either with implants or with your own tissue as an auto-augmentation breast lift.
Fat transfer after breast implant removal
Fat grafting is the natural companion to breast implant removal (explant). Once the implants are out, fat can soften the transition at the top of the breast and restore some of the volume lost, so the breast looks less empty. When the skin has been stretched by the implant or the nipple sits low, fat is combined with an explant-lift rather than used alone. Fat cannot replace a large implant in one session, and this is discussed plainly at the consultation — see also will my breasts sag after implant removal?.
Weight loss, Ozempic and fat transfer
Fat transfer needs fat. After major weight loss — including weight loss with GLP-1 medicines such as semaglutide (Ozempic, Wegovy) or tirzepatide (Mounjaro) — many patients have little fat left to harvest, and the breast skin is often too loose for fat alone to restore shape. In that situation a lift, with or without an implant, is usually the better plan; the changes are explained on the “Ozempic breasts” page.
Surgery is planned once your weight has been stable for several months. GLP-1 medicines are usually paused about two weeks before surgery, as directed by the anaesthesia team — never stop or change them on your own.
Breast imaging and screening
Grafted fat can form small oil cysts or calcifications that appear on mammograms and ultrasound. Radiologists are generally able to tell these apart from worrying changes, but they need to know you have had fat grafting. Depending on your age and history, breast imaging is arranged before surgery, and you continue your normal breast screening afterwards. Any new lump that appears later should be checked rather than assumed to be grafted fat.
Pain and recovery
Ultrasound-guided nerve blocks keep the first days comfortable. Most patients find the donor area — sore, bruised and swollen, as after liposuction — more noticeable than the breasts.
- Day of surgery — walking the same day; short hospital stay as advised
- Week 1 — rest at home; Manual Lymphatic Drainage (MLD) on the donor areas starts to reduce swelling
- About 1 week — most patients return to desk work
- Weeks 3–4 — light cardio
- Week 6 — compression garment on the donor areas stopped; full exercise
- Months 3–6 — swelling resolves and the retained volume becomes clear
On the breasts, firm pressure is avoided in the early weeks so the graft can settle — you will be told which soft support bra to wear and how to sleep. In the UAE climate, stay in air-conditioned spaces while wearing the garment.
What are the risks?
Besides the general risks of surgery and anaesthesia — bleeding, infection, blood clots — the specific risks are:
- Reabsorption greater than expected, leaving less volume
- Fat that does not survive, forming firm lumps or oil cysts that may need imaging or, occasionally, treatment
- Asymmetry in the amount retained on each side
- Donor-site irregularity, as with any liposuction
Conservative grafting in fine layers, careful donor-site technique and realistic planning are what keep these risks low.
Patients from Dubai
Many of Dr. Michels’ breast patients live in Dubai. All consultations and surgery take place at Elyzee Hospital in Abu Dhabi — you can book by WhatsApp or phone on +971 50 106 7981, or through the contact form. The pre-operative tests, surgery and follow-up visits are grouped where possible to keep the number of journeys down. See also breast augmentation for patients in Dubai.
What does your investment include?
Fat transfer breast augmentation fees are quoted individually after consultation, because the number of donor areas, the volume to be grafted and any combined procedure determine the operating time and hospital stay. The quote is all-inclusive:
- Surgeon’s fee and pre-operative planning
- Specialist anaesthesiologist, including ultrasound-guided nerve blocks
- Hospital stay and operating theatre at Elyzee Hospital
- Post-operative compression garments
- Discharge medication
- Follow-up appointments and ongoing clinical support
Questions, answered
Fat Transfer Breast Augmentation in Abu Dhabi — your questions
What is fat transfer breast augmentation?+
Fat transfer breast augmentation — also called breast fat transfer, fat grafting to the breast or natural breast augmentation — increases breast volume using your own fat instead of an implant. Fat is removed by liposuction from an area such as the abdomen, flanks or thighs, processed to keep the healthy fat cells, and injected in many small threads into the breast. The result is soft, moves naturally and contains no foreign material.
Who is a good candidate for breast augmentation without implants?+
The best candidates want a modest, natural increase rather than a dramatic change, have enough fat in at least one donor area, have reasonably firm skin and a breast that is not significantly sagging, and are at a stable weight. They also accept that the final volume is less predictable than with an implant and that a second session may be needed to reach their goal.
How much bigger can my breasts get with fat transfer?+
The gain is modest. The amount of fat a breast can safely receive in one session is limited by how much the existing skin and tissue can accommodate, and part of the transferred fat does not survive. Fat transfer is designed for a natural, subtle increase and for improving shape — fuller upper breast, softer cleavage — not for a large change in cup size. If you want a substantial increase, an implant or a hybrid augmentation is usually more appropriate.
Is some of the transferred fat reabsorbed?+
Yes. Not every grafted fat cell establishes a new blood supply, and a portion of the transferred fat is reabsorbed by the body over the first months. This is expected and is part of the planning. The volume you see in the first weeks includes swelling and fat that will not survive; the result is judged once it has settled, after several months.
Will I need more than one fat transfer session?+
Some patients do. Because the amount that can be grafted safely at one time is limited, and some fat is reabsorbed, women who want more volume than a single session can give may choose a second session once the first result has settled, usually after several months. Whether a second session is likely in your case is discussed honestly at the consultation.
Do I have enough fat for fat transfer breast augmentation?+
You need enough fat in at least one donor area — commonly the abdomen, flanks, back or thighs — both for the graft and to allow for the portion that is lost during processing and reabsorption. Very slim patients may not have enough to make a meaningful difference. The donor areas are examined at the consultation, and if there is not enough fat, other options are discussed.
How is the fat harvested?+
The fat is collected by gentle liposuction through small access points, using techniques that protect the fat cells — VASER ultrasound-assisted liposuction or conventional suction, depending on the donor area. The donor area is contoured at the same time, which many patients see as a welcome secondary benefit. The fat is then processed to separate viable fat from fluid and oil before it is transferred.
How is the fat placed in the breast?+
Through small entry points, the purified fat is injected with fine cannulas in many small threads, at different depths and in different directions, rather than as a single mass. Distributing the fat this way gives each portion contact with surrounding tissue and blood supply, which is what allows it to survive. Injecting too much in one place increases the risk of fat that does not survive and forms firm lumps or oil cysts.
Fat transfer vs implants: which is better?+
Neither is better for everyone. Implants give a larger, predictable and immediate volume, but they are a foreign body that may need future revision. Fat transfer gives a smaller, softer and entirely natural gain with no implant, but the final volume is less predictable and a second session may be needed. The choice depends on how much volume you want, your tissue and your preference.
What is the difference between fat transfer and hybrid breast augmentation?+
Fat transfer breast augmentation uses fat alone. Hybrid breast augmentation combines a smaller implant, which provides the main volume and projection, with a layer of fat over it to soften the edges and the upper breast. Hybrid suits women who want more volume than fat alone can give but a more natural look and feel than an implant alone, particularly slim patients with thin coverage.
Can fat transfer lift sagging breasts?+
No. Fat adds volume; it does not raise a nipple that sits below the breast fold or remove stretched skin. If the breasts sag, a breast lift is needed, and fat transfer can be added to soften the upper breast. For women who want fullness without any implant, an auto-augmentation breast lift, which reuses the lower breast tissue, is another option.
Can I have fat transfer after breast implant removal?+
Yes, and this is one of its most common uses. After an explant, fat grafting can soften the upper breast and restore some of the volume lost, alone or together with a breast lift. It cannot replace a large implant in one session, so expectations are set carefully. The options are explained on the breast implant removal (explant) page.
Does fat transfer to the breast affect mammograms?+
Grafted fat can create small oil cysts or calcifications that show up on breast imaging. Radiologists are generally able to distinguish these from worrying changes, but you must tell them that you have had fat grafting to the breasts. Depending on your age and history, breast imaging is arranged before surgery, and you continue your normal breast screening afterwards.
Is fat transfer to the breast safe?+
When planned and performed carefully, it is an established procedure. Its specific risks are fat that does not survive and forms firm lumps or oil cysts, asymmetry in the amount retained, contour irregularity at the donor site, and the general risks of surgery and anaesthesia. Spreading the fat in fine layers and not over-filling the breast are the main ways these risks are reduced.
I lost weight with Ozempic or Mounjaro. Can I have fat transfer?+
Possibly, but fat transfer needs fat. After major weight loss with GLP-1 medicines, many patients have little fat left to harvest, and the breast skin is often too loose for fat alone to restore shape — a lift, with or without an implant, is frequently more suitable. Surgery is planned once your weight has been stable for several months, and GLP-1 medicines are usually paused about two weeks before surgery as directed by the anaesthesia team. Never stop them on your own.
What if I lose or gain weight after fat transfer?+
Transferred fat that has survived behaves like fat elsewhere in your body: if you gain weight, the grafted breasts may become larger, and if you lose significant weight, they may become smaller. A stable weight before and after surgery protects the result.
How long does the operation take?+
It depends on the volume of fat harvested and the number of donor areas, but fat transfer to the breasts usually takes a few hours under general anaesthesia. It is performed at Elyzee Hospital in Abu Dhabi, with ultrasound-guided nerve blocks placed by the anaesthesiologist for comfort after surgery.
What is recovery like after fat transfer breast augmentation?+
The donor area usually feels like the recovery after liposuction — soreness, bruising and swelling — and is often the more noticeable of the two. The breasts are tender and swollen for some days. Most patients return to desk work within about a week, wear a compression garment on the donor area for around six weeks, and avoid strenuous exercise for several weeks. Pressure on the breasts is avoided early on so the graft can settle.
Do I need to wear a bra or compression after breast fat grafting?+
A compression garment is worn over the liposuction donor areas for around six weeks. On the breasts, firm pressure is avoided in the early weeks because it can compromise the grafted fat; you will be told exactly which soft support bra to wear and when. Manual Lymphatic Drainage (MLD) on the donor areas helps reduce swelling.
How long do the results of fat transfer to the breast last?+
Fat that has survived after several months is generally considered permanent. It will age with the rest of the breast and change with your weight, pregnancy and time, as natural breast tissue does. There is no implant to replace, so there is no device-related revision cycle.
Can fat transfer correct asymmetry or small defects?+
Yes. Fat is very useful for fine adjustments: adding a little more volume to one breast, softening a contour irregularity or filling a localised hollow. For major size differences, an implant or a combined operation may be needed, but fat is often used to fine-tune the final result.
How much does fat transfer breast augmentation cost in Abu Dhabi or Dubai?+
The cost depends on the number of donor areas, the volume of fat to be grafted, whether a lift or another procedure is combined and the hospital stay. Fees are quoted individually after the consultation, once the plan is defined. The quote is all-inclusive — surgeon, anaesthesiologist with nerve blocks, hospital and theatre, garments, discharge medication and follow-up. No fixed price list is published.
Do you perform fat transfer breast augmentation for patients from Dubai?+
Yes. Many of Dr. Michels' breast patients live in Dubai. All consultations and surgery take place at Elyzee Hospital in Abu Dhabi, and the pre-operative tests, surgery and follow-up visits are grouped to keep the number of journeys to a minimum.
